Cash, check or money order paid at the beginning of the session, unless other arrangements have been made. Credit cards also accepted with an additional processing fee.
I have a 24 hour cancellation policy and request that you notify me via phone (303) 817-4330 if you are unable to keep your scheduled appointment. Appointments not canceled within 24 hours, will be charged the full rate to your credit card.
Your health insurance or employee benefit plan may cover my services in full or in part. I will provide a monthly statement upon request for you to submit to your insurance company. Please carefully check your coverage to answer the following questions:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much will my plan cover for an out-of-network provider?
- How much will it cover per therapy session?
- Do I need approval from my primary care physician?
